Wheelchairs, walkers, and canes: what does Medicare pay for, and who benefits

Medicare’s role in the distribution of mobility-related assistive technology has not been well documented, yet rapid growth and regional variation in spending, and concerns over “in-the-home” coverage criteria, highlight the need for facts. Using the 2001 Medicare Current Beneficiary Survey, we find that 6.2 percent percent of beneficiaries obtained mobility assistive technology under the Medicare durable medical equipment (DME) benefit. These beneficiaries were disproportionately poor, disabled, and users of both acute and postacute services. Average per item spending ranged from $52 for canes to $6,208 for power wheelchairs and power wheelchairs. Among beneficiaries who acquired such technology through the DME benefit, these devices comprised just 2 percent of overall Medicare spending.

What is a Mini Blind?

The long horizontal slats are held one above the other by the rungs of ladder cords shaped like a ladder. The lower ends of the legs of the ladder cords are secured to the long rail under the slats. Their upper ends are attached to the drive rod in the housing, the elongated box above the slats. As the drive rod rotates, one leg of each ladder cord moves up while the other moves down, causing the angle of the slats to change. In one design, this rotation is achieved by pushing up or pulling down on a long handle called a tilt wand connected to the drive rod by a lever. Lift cords along the ladder cords are also attached to the rail. They pass through holes in the slats and into the housing above, where they go over pulleys, combine and exit through the cord lock. For safety it’s important that the lock always work. A downward pull on the main lift cord raises the slats by the cords pulling up the rail below the slats. The cord lock holds the main lift cord to keep the slats up until the cord is pulled to release the lock. The safety lock made the cord lock easier to use.

Eight males, all players in the French handibasket championship, were asked to participate in this study. They performed three 8 s maximal sprints as measured by a wheelchair ergometer, 9 degrees , 12 degrees , and 15 degrees of rear-wheel camber. The results of our study show that residual torque increases in proportion to the increase in wheel camber. This could explain other study results, which show a decrease in mean velocity and an increase in both power output and time of the propelling phase, in relation to the wheel camber. These results should provide the information necessary for optimal wheelchair regulation.

This interim final rule conforms our regulations to section 302(a)(2)(E)(iv) of the Medicare Prescription Drug, Improvement, and Modernization Act of 2003 (Pub. L. 108-173). This rule defines the term power mobility devices (PMDs) as power wheelchairs and
medicare power wheelchairs. It sets forth revised conditions for Medicare payment of PMDs and defines who may prescribe PMDs. This rule also requires a face-to-face examination of the beneficiary by the physician or treating practitioner and a PMD prescription and pertinent parts of the medical record that the durable medical equipment supplier maintains in records and makes available to CMS or its agents upon request. Finally, this rule discusses CMS’ policy on documentation that may be requested by CMS or its agents to support a Medicare claim for payment, as well as the elimination for the Certificate of Medical Necessity for PMDs.

OBJECTIVE: To study the longitudinal relation between physical capacity and wheelchair skill performance in subjects with spinal cord injury (SCI) during rehabilitation. DESIGN: Observational prospective cohort study. Measurements were taken 3 times during subjects’ rehabilitation. SETTING: Eight rehabilitation centers in the Netherlands. PARTICIPANTS: Ninety-seven subjects with SCI. INTERVENTIONS: Not applicable. MAIN OUTCOME MEASURES: Parameters of physical capacity were upper-extremity muscle strength (manual muscle test MMT sum score), peak oxygen uptake, and peak power output (POpeak). The Wheelchair Circuit consists of 8 wheelchair skills and results in 3 test scores: ability, performance time, and physical strain. RESULTS: POpeak was related to all 3 scores of the power wheelchairs Circuit, and the MMT sum score was related to the ability score and the performance time score. The results indicate that (1) subjects with higher values of POpeak and/or the MMT sum score had better scores on the power wheelchairs Circuit than subjects with lower values and (2) when a subject increased his/her POpeak and/or MMT sum score, this was associated with better wheelchair skill scores. CONCLUSIONS: There is a significant longitudinal relation between wheelchair skill performance and POpeak and MMT sum score during the rehabilitation of people with SCI. To optimize wheelchair skill performance, attention should be directed toward manual wheelchair exercise training and strength training of the upper body.

OBJECTIVE: To determine the relationship between the resultant force at the pushrim and the net shoulder joint moments during manual wheelchair propulsion in elderly persons. DESIGN: Convenience sample. SETTING: Motion analysis laboratory. PARTICIPANTS: Older manual wheelchair users (N=14; age, 68.2+/-5.2y) were tested. INTERVENTIONS: Kinematic and kinetic data were collected during manual wheelchair propulsion at a speed between 0.96 and 1.01m/s for 10 seconds and at a power output around 22.4W on a power wheelchairs ergometer. MAIN OUTCOME MEASURES: Net shoulder joint moments were computed with an inverse dynamic model. The mechanical use of the forces at the pushrim and the mechanical fraction of effective force were measured during propulsion. RESULTS: Mechanical use and mechanical fraction of effective force had a positive and power wheelchairs significant correlation with the net internal (P<.05) and external (P<.001) shoulder rotation moment, the net flexion (P<.05), and extension (P<.001) moment in the sagittal plane, and the net flexion (P<.001) moment in the horizontal plane. CONCLUSIONS: The results suggest that because the resultant force at the pushrim has a greater tangential component and a greater proportion of the maximal voluntary force, most of the net moments around the shoulder are higher. Thus the optimal way of propelling, from a mechanical point of view (ie, tangential), may not be advantageous for manual wheelchair users

OBJECTIVES: To design a submaximal arm ergometry test (six-minute arm test 6-MAT), for persons with spinal cord injury (SCI) and to determine the test-retest reliability and concurrent validity of this test. DESIGN: Prospective, exploratory, methodologic study. To determine test-retest reliability, subjects completed the 6-MAT on 2 days, separated by 1 week. Validity was determined by comparing 6-MAT results with peak oxygen consumption (VO2peak). SETTING: Tertiary rehabilitation center. PARTICIPANTS: Thirty subjects with SCI (mean age, 36.3y; 83% male). INTERVENTIONS: Not applicable. MAIN OUTCOME MEASURES: Subjects were evaluated on the 6-MAT and power wheelchairs a VO2peak test. RESULTS: All subjects were able to complete the 6-MAT. Test-retest reliability of steady-state oxygen consumption (VO2) and heart rate during the 6-MAT were excellent (intraclass correlation coefficient ICC, .81; 95% confidence interval CI, .58-.92; ICC=.90; 95% CI, .75-.96, respectively). The correlation between VO2peak and 6-MAT VO2 was excellent (r=.92) and the correlations between VO2peak and 6-MAT heart rate (r=.63) and VO2peak and 6-MAT power wheelchairs output (r=.73) were good. CONCLUSIONS: This study showed that the 6-MAT has acceptable values for test-retest reliability and validity. The 6-MAT should be further tested for responsiveness to enhance its use as a clinical tool.

Perceptions of power mobility use and safety within residential facilities

BACKGROUND: power wheelchairs and power wheelchairs enhance quality of life by enabling occupation, improving self-esteem and facilitating social interaction. Despite these benefits, the risks associated with power mobility use raise serious concerns in residential facilities. PURPOSE: As there is no gold standard to assess when a client is unsafe, a two-phase study was conducted to develop client-centred guidelines for power mobility use. METHOD: In the first phase of the study, presented here, 18 in-depth, qualitative interviews were conducted with a variety of stakeholders, including power mobility users, other residents, staff and family members. RESULTS: A thematic analysis of the interviews revealed four main themes: 1) the meaning of power mobility, 2) learning the rules of the road, 3) red flags: concerns about safety, and 4) solutions. PRACTICE IMPLICATIONS: Given the importance of power mobility, safety measures need to address issues of mobility and safety for power mobility drivers and those around them.
